If you or a family member start recognizing memory changes in yourself or a loved one, we would like to help.
At the Memory Clinic, our interdisciplinary team will walk you through a comprehensive assessment. Our team is comprised of a physician, a registered nurse, a social worker, and very supportive members of community agencies including Home & Community Care Support Services South West and the Alzheimer Society of Perth County.
You can expect assessments on many aspects of memory function including driving safety. You and/or your family member will receive recommendations on treatment options and plans for care.
We aim to help patients with memory difficulties maintain independent living and the best quality of life for as long as possible.
The Memory Clinic is offered monthly rotating between the three locations of the STAR Family Health Team. Please speak with your family doctor or Nurse Practitioner to discuss if this clinic is an option for you.
