Patients are also able to get their bloodwork done at the Jenny Trout Centre in Stratford.

As many of you know, OHIP (Ontario Health Insurance Plan) covers the costs of most of your medical needs. However, there are many services we provide which are not covered by OHIP. These services can require significant time and resources to provide.

Examples of these include, but are not limited to:

• Prescription Renewals Without a Visit
• School / Camp Forms
• Referrals for Physiotherapy, Massage Therapy, Orthotics, etc without a visit.
• Insurance Forms

We have now partnered with PatientSERV, which offers you the choice of a yearly plan to allow for cost savings or ease of payment for a pay-as-you-go option.

Prescriptions for medication are written by your doctor. If you have a question about your medication, please call the office at 519-655-2322. Be sure to have the prescription label handy to reference during the call. For prescription renewals, please have pharmacy fax a request to your family doctor.

Referrals to specialists as physicians deem necessary and appropriate.

There are currently no resident physicians at this location.

Patients are notified of all test results either by phone or email.
