After Hours Clinics are to be utilized for urgent medical problems that arise after your clinic has closed.

Here are your Patient Responsibilities.

Health Card:
Valid and with You.

Please bring your health card to every visit and make sure that it is a valid health card. If you need to renew your health card, Service Ontario is located at 5 Huron Street in Stratford, behind the Court House. No appointment is needed, but ID is required for each renewal. Please visit the Service Ontario Website for more information.

IMPORTANT NOTE: You will be charged for your clinic visit if you have been informed once by our front staff about an expired health card.

To better serve your health needs, it is important for you to keep all your personal information current in our records. Please inform reception of any changes to your address, home, work and/or cell phone number(s), email address and pharmacy you use for your prescription needs.

Clothing
for Examinations

Please dress appropriately for the area being examined. For example, if you are being seen for a sore shoulder, please wear a top that exposes the shoulder, or change into a gown provided in each exam room.

This will assist the clinician in providing a thorough assessment of the problem.
